For When Emotions Arrive at Full Volume and There Is No Dial

Some people feel things at an intensity other people do not seem to.

An offhand comment lands like an accusation. A delay in a reply becomes evidence of something. Anger arrives fully formed, and by the time you notice it, you have already said the thing you will be apologizing for tonight. Afterwards comes the familiar shame and a private theory that there is something fundamentally wrong with you.

There usually is not. What there often is is a nervous system that reacts faster and harder than average and a set of skills that nobody ever taught you.

What Is Dialectical Behaviour Therapy?

DBT was developed for people who experience emotions very intensely and struggle to regulate them. It grew out of cognitive behavioural therapy and shares a practical, skills-based character.

The word "dialectical" points to the central idea, which is holding two apparently opposing truths at once.

You are doing the best you can with what you have. And some things need to change.

Most approaches to intense emotion pick one of those and fail. Pure acceptance leaves someone stuck. Pure change leaves them feeling criticized for something they did not choose. DBT insists on both, and that balance is what makes it work for people who have found other approaches either too soft or too blaming.

An Honest Note About What We Offer

This matters, because people searching for DBT are often looking for something specific.

Full DBT is a structured program. It typically involves weekly individual therapy, a separate skills training group, between-session phone coaching, and a consultation team supporting the therapists. It runs to a defined curriculum over a set period, and it is usually what a psychiatrist or hospital team means when they recommend DBT.

We do not currently offer a full DBT program. There is no skills group here.

What we do offer is DBT-informed individual counselling. Two of our clinicians draw on DBT skills and principles within one-to-one work. For many people, this is genuinely useful. For someone who has been specifically referred to a DBT program, it is not the same thing, and we would rather say so now than at the end of a phone call.

If you are unsure which one you need, please bring it up during the consultation, and we will give you a clear answer, including directing you elsewhere if that is the right option.

The Four Skill Areas

DBT teaches skills in four areas. Within DBT-informed individual counselling, a therapist uses the same skills in a less formal way than in a structured program.

Mindfulness

Noticing what is happening as it happens, rather than being inside it before you realize it. This skill is the foundation, because you cannot use any other skill until you have noticed you need one.

Distress tolerance

Distress tolerance means getting through a moment of acute distress without doing the thing that makes tomorrow worse. Not solving the problem, not feeling better about it. Just getting through the next twenty minutes intact.

Emotion regulation

Understanding what an emotion is doing, reducing vulnerability to overwhelming ones, and changing the conditions that make them more likely. Sleep, food, and alcohol do more here than people expect.

Interpersonal effectiveness

Asking for what you need, declining things, and handling conflict without either capitulating or detonating. Many people who feel intensely have learned to swing between the two.

DBT Counselling for Stress and Emotional Distress

What Can DBT-Informed Work Help With?

Depending on your circumstances and your therapist's clinical assessment, these strategies may support people experiencing:

  • Anxiety, worry, and persistent overthinking
  • Depression and low mood
  • Chronic stress and burnout
  • Avoidance that has narrowed daily life
  • Harsh self-criticism
  • Grief and loss
  • Chronic pain and long-term health conditions
  • Difficult experiences from the past that continue to intrude
  • Feeling stuck or disconnected from what used to matter
  • Living with a long-term condition or diagnosis

It is often useful for people who have already tried to think their way out of something and found that understanding it did not change it.

ACT is not automatically right for everyone, and a therapist should assess that rather than assume.

What Might Happen During a Session?

Expect practical work alongside conversation.

You might spend time identifying exactly where in a sequence things tip over, since the useful intervention point is usually earlier than people assume. You will likely learn specific skills and be asked to practice them between sessions. Some clients keep a simple record of emotional intensity and what helped, because patterns are much easier to see written down.

There is usually a strong emphasis on what worked rather than what went wrong, which many people find unfamiliar at first.

Is This Program Right for Me?

It may suit you if you do:

  • Feel emotions more intensely than people around you seem to
  • Act on feelings quickly and regret it afterwards
  • Have found approaches that focus purely on thinking differently do not reach the problem
  • Want concrete skills rather than insight alone
  • Are tired of being told to calm down by people for whom calming down is straightforward

It may suit you less if you have been referred to a full DBT program with a skills group or if what you are dealing with is better served by something else. Your therapist will give you an honest view.

Frequently Asked Questions

Is this full DBT?

No. We offer DBT-informed strategies in individual counselling. Full DBT is a structured program including a skills group, which we do not currently provide.

My doctor referred me for DBT. Is this what they meant?

Possibly not. If you were referred to a DBT program specifically, tell us at the consultation, and we will be honest about whether we are the right fit.

Is DBT only for borderline personality disorder?

It was originally developed for that, but the skills are now widely used for anyone who experiences emotions intensely, regardless of diagnosis. You do not need a diagnosis to benefit from them.
